Send documents to your provider
Tap More on the bottom navigation bar.
If you don't see the navigation bar, tap X in the upper left to close the page you're on.
Tap Documents.
Upload documents
Upload a document by tapping Add Document at the bottom of the page.
Choose the provider from the dropdown. This is the provider your document will be sent to.
Add a title and description for your document.
Tap the + to choose the document to upload.
Select a document from your device. Tap Save to add your document. Your provider will automatically be notified that there is a document for them to review.
Tap the X in the upper left at any time to cancel uploading your document and return to the previous page.
The document will have a status of Pending until the provider has reviewed the document.
Once the provider has reviewed and accepted the document, the status will change to Accepted.
Filter documents
Filter your documents by selecting a start date and end date, then tap Search. The default filter is for the past two years.
Tap the checkbox for Include deleted to choose whether deleted documents will show in the list.
When this is selected, deleted documents will have a status of Deleted in the upper right of the card.
Tap the magnifying glass to search for a document by title.
Sort by
Selecting Sort by allows you to change how the form list is ordered.
Tap the option you'd like to sort by, or tap Cancel to return to the list.
- Date
- Sorts the document list by the date they were added. Newer documents are at the top of the list.
- Name
- Sorts the document list alphabetically by their titles.
- Provider
- Sorts the document list by the last name of the provider they were sent to.
Viewing documents
Preview your document by tapping on the file name of the document.
Tap X in the upper left to close the preview.
Remove documents
To remove your document from the documents list, tap the three dots in the upper right of the card.
Documents that have already been deleted will show Deleted as their status and will not include the three dots option.
Tap Delete to remove the document, or tap Cancel to close the actions menu.
Tap Delete to confirm you want to remove the document.
Uploaded documents from the practice cannot be deleted. If you need a document deleted, contact your provider.
Consent Forms
Sign consent forms
If you have pending consent forms, you'll see a notification on the dashboard. Tap on the notification to go straight to the consent forms page.
Tap Sign to view and complete a consent form.
Review the form, then tap the checkbox to indicate that you agree to sign electronically. Enter your name in the text box and tap Sign to complete the form. Your provider will automatically be notified that you've signed the form.
Tap the X in the upper left at any time to cancel signing the form and return to the previous page.
View consent forms
Tap on More in the bottom navigation bar.
If you don't see the navigation bar, tap X in the upper left to close the page you're on.
Tap Documents.
Tap Consent forms to go to the section.
You may need to swipe left to see the consent forms tab at the top of the page.
Sort by
Tap Sort by to change how the form list is ordered.
Tap the option you'd like to sort by or tap Cancel to return to the list.
- Date
- Sorts the form list by the date they were sent. Newer forms are at the top of the list.
- Name
- Sorts the form list alphabetically by their titles.
- Provider
- Sorts the form list by the last name of the provider who sent it.
View completed forms
Completed consent forms are shown in the list without the Sign button. Tap View in the upper right to view the form.
Tap X in the upper left to close the form view.
Download completed forms
From the form view, tap the download icon in the upper right. This will download the PDF of the signed form to your device.
Intake Forms
If you have pending intake forms, you'll see a notification on the dashboard. Tap on the notification to go straight to the intake forms page.
Tap Complete to open and complete an intake form.
Add additional information
Tap on a field to add information to it. Swipe up to scroll through the intake form.
Your provider's office creates each intake form, so the information you're asked to complete depends on what your provider wants to know before your appointment.
Tap Cancel at the bottom of the form to cancel completing the intake form and discard the information you entered. The intake form will remain in your Requires action list.
Save and finish later
Tap Save & Finish Later to save your progress and complete the intake form at another time. Your information will be saved and the form will remain in your Requires action list.
Submit a form to your provider
Tap Complete to save your information and complete the form. The form will be moved to the Completed section and your provider will automatically be notified that you've completed the form.
View Intake Forms
Tap on More in the bottom navigation bar.
If you don't see the navigation bar, tap X in the upper left to close the page you're on.
Tap Documents.
Tap Intake forms to go to the section.
You may need to swipe left to see the intake forms tab at the top of the page.
Sort by
Tap Sort by under the Requires action or Completed headings to change how the form list is ordered in that section.
Tap the option you'd like to sort by, or tap Cancel to return to the list.

View a completed form
Tap View on a form in the Completed section of the page.
You won't see this section if you haven't completed any intake forms yet.
Tap X in the upper left to close the form view.
Download completed forms
From the form view, tap the download icon in the upper right. This will download the PDF of the completed form to your device.
