Encounter notes are available in the patient portal to support transparency and patient engagement in care. This allows patients to:
- Review what was discussed during their visit (diagnoses, care plans, medications, and next steps)
- Better remember and understand their provider’s recommendations
- Track their health and progress over time
- Prepare questions for future appointments
- Share accurate information with caregivers or family members if desired
Providing access to encounter notes helps improve communication and supports informed, collaborative care.
Access your encounter notes
From the More tab, select Encounter Notes.
Tap view file.
Export and save file
Tap the download icon in the top right corner.
