Documents in the MyRXNT app

The MyRXNT app provides a secure place for you to upload, store, and view important health-related files in one convenient location. You can add documents directly to your portal and easily access any files shared with you by your provider’s office—helping keep your health information organized, accessible, and available when you need it. Documents may also include consent and intake forms, if applicable to your provider's office.

Upload and manage documents

From the dashboard, tap More.

Tap Documents.

All documents uploaded by you or your provider's office will be displayed here. Tap View to view any document.

Upload documents

Tap Add Document at the bottom of the page.

Please select your practice's location (if applicable) and the provider you'd like to send your document to. Then, enter a title and description for your document.

Tap the + to select the document you want to upload from your device. When you're ready, tap Save to add your document. Your provider will be notified that a document is ready for them to review.

You will be taken back to the Document page where the new document will show a status of Pending until the provider reviews it. After the provider has reviewed and approved the document, the status will update to Accepted

Search documents

Use the Search field to enter a document title, or use the date fields to find a document by the date it was uploaded, or you can use both to narrow down your search.

Tap the Include deleted checkbox to choose whether deleted documents will show in the list.

When this is selected, deleted documents will have a status of Deleted in the upper right of the card. Tap the magnifying glass to search for a document by title.

Sort documents

Choosing Date lets you change the order of the list. 

 The sorting works the same way for both consent and intake forms.

Tap the option you'd like to sort by.

  • Date: Sorts the document list by the date they were added. Newer documents are at the top of the list.
  • Provider: Sorts the document list by the last name of the provider they were sent to.
  • Name: Sorts the document list alphabetically by their titles.

View documents

Preview your document by tapping View.

Delete documents

To remove your document from the documents list, tap Delete.

 Documents that have already been deleted will show Deleted as their status and will not include the three dots option.

Tap Delete to confirm you want to remove the document.

Uploaded documents from the practice cannot be deleted. If you need a document deleted, contact your provider. 

Consent forms

Sign consent forms

If you have pending consent forms, you'll see a notification on the dashboard. Tap on the notification to go straight to the consent forms page.

Tap Sign to view and complete a consent form.

Review the form, then tap the checkbox to indicate that you agree to sign electronically. Enter your name in the text box and tap Sign to complete the form. Your provider will automatically be notified that you've signed the form.

View consent forms

Within Documents, tap the Consent forms tab.

If you have any consent forms that aren't finished yet, just tap Sign to continue working on them.

View completed forms

You can see completed consent forms in the list with the View button. Just tap it to take a look.

Download completed forms

From the form view, tap the download icon in the upper right. This will download the PDF of the signed form to your device.

Intake forms 

If you have pending intake forms, you'll see a notification on the dashboard. Tap on the notification to go straight to the intake forms page.

Tap Complete to open and complete an intake form.

 Your provider's office creates each intake form, so the information you're asked to complete depends on what your provider wants to know before your appointment.

Scroll through to complete the form. You'll have a few options at the end:

  • Tap Cancel to discard your input; the form stays in your Action Required list.
  • Tap Save & Finish Later to save progress and complete the form later; it remains in your Requires action list.
  • Tap Complete to save and submit the form; it moves to Completed, and your provider is notified.

View intake forms

In Documents, go to the Intake Forms tab.

If you have any intake forms that aren't finished yet, just tap Complete to continue working on them.

View a completed form

Tap View on a form in the Completed section of the page.

 You won't see this option if you haven't completed any intake forms yet.

Download completed forms

From the form view, tap the download icon in the upper right. This will download the PDF of the completed form to your device.
