About
Build Custom Templates or choose from Preset Templates to structure your Ambient IQ AI-generated notes. Both options help ensure accurate, consistent documentation with minimal manual effort.
Within Utilities, expand Clinical Admin Tools, then scroll to Ambient IQ Templates.
You must have the Clinical Administrator permission to access this tool.
You'll see two tabs:
- Custom Templates: Allow practices to tailor how AI-generated notes are structured. By defining the sections and clinical details Ambient IQ listens for, notes are automatically organized to match your workflows, documentation requirements, and specialty needs—reducing manual edits after the visit.
- Preset Templates: Ready-made note structures built by RXNT for common visit types and specialties. These templates come preconfigured with standard components and clinical elements—such as the Psychiatric Intake Note—allowing practices to get started quickly without customization while still benefiting from accurate, well-organized AI-generated notes.
Custom templates
You can create custom templates in two ways: either start with a preset and modify the sections or components to fit your needs, or build a template from scratch for complete customization.
Build off of a preset
Click Create Template.
By default, Start from a Template Preset is selected, so you can leave this option as is. Then, pick a Template Preset from the dropdown menu below. After that, choose a specialty from the available options and click Create Template.
Step 1: Template
Assign a Template Name.
All existing components of the preset template you chose will be organized on the right side. All additional available components will be in the panel on the left.
Build your template by dragging and dropping components in the desired arrangement.
Use the Search field to quickly locate a desired component.
Once found, drag your new component over to your template.
Remove headers and/or components using the X.
To add a new section, simply click and drag Add Header into the template builder. Then, enter the title for your new header.
Once all sections and components are arranged to your liking, click Next.
Step 2: Settings and preview
Check out your template preview on the right. On the left, you'll see the available settings, which include:
- Include Component Headers: If this is toggled on (blue), each component's name will appear on the template. If it's toggled off (gray), the component names won't be shown. Please note that component headers are different from section headers—section headers will still be visible.
- Include Empty Sections: If this is toggled on (blue), the component will still appear on the template after a visit, even if there's no data for it. If it's toggled off (gray), any components without data after the Ambient session won't show up on the template.
You can use the Global Default Settings to apply these options to the whole template. Alternatively, you can adjust the Component Settings below to toggle settings on or off for individual components.
When you make changes to the settings, a confirmation pop-up will appear to let you know the settings were applied appropriately.
If you're happy with your preview, click Save Template to complete. If you'd like to make any changes, click Edit Template to return to the first step.
Build from scratch
Click Create Template.
Please select the Build from Scratch radio button, pick a Specialty from the list, and then click Create Template.
Assign a Template Name.
To create a section, simply click and drag Add Header into the template builder. Then, enter the title for your header.
All available components will be in the panel on the left. Build your template by dragging and dropping components in the desired arrangement.

Use in practice
When you start a new ambient session, you'll find your custom templates in the Encounter Form dropdown, listed under the name you chose.
Proceed with your visit as you normally would. Once you're done, you can access your encounter in the web-based version and review it as usual.
Manage custom templates
Click Preview to view a template (with sample patient data) so you can see how it will look when you use it.
To edit a template, click the pencil icon. To remove a template, click the garbage can icon.
You can use the checkboxes to add or remove templates from your personal favorites or the group's favorites. Favorites will appear in the Encounter Form dropdown menu when you start a new ambient session.
Custom templates created from an existing preset will show which base template they were built on.
Preset templates
Click the Preset Templates tab to review and manage available preset templates.

Click Preview to view a completed preset template (with sample patient data) so you can see how it will look when you use it.
Additionally, click on a preset template name below to view the list of components it includes.
