Overview
Navigate to the intended patient's dashboard and click Chart Summary from the navigation menu.
When accessing the Chart Summary navigation from a patient’s dashboard, the following information will be displayed by default:
Filtering Options
To filter CCD information, click the SEND button on the right-hand side and select one of the options from the dropdown: View, Fax, Download CCD, or Email CCD. Depending on your selection, a different pop-up window will open, which functions as the control center for filtering PHI on the final document.
View
Select one of the Summary Options, set a Date Range, and deselect/select which USCDI Elements you’d like to exclude/include. If things look correct, click Continue to proceed.
The CCD will open after you click continue.
Fax
Select one of the Summary Options, set a Date Range, select a specific Fax Option, and deselect/select which USCDI Elements you’d like to excldue/include. If things look correct, click Continue to proceed.
The Fax popup will open after you click Continue. Complete the To and Attention To fields, and include a Cover Sheet or Fax File if necessary. Once complete, click Send.
Download CCD
Select one of the Summary Options, set a Date Range, and deselect/select which USCDI Elements you’d like to exclude/include. If things look correct, click Download to download a ZIP file onto your computer.
The Chart Summary will display a CCD Zip File Password unique to the file. Copy the password, which is required to open the zip file (found in your computer's downloads).
Email CCD
Select one of the Summary Options, set a Date Range, and deselect/select which USCDI Elements you’d like to exclude/include. If things look correct, click Continue to proceed.
The Message pop-up window will open after you click Continue. Complete the From, To, Subject, and Message fields, and include an Attachment if necessary. Once complete, click Send Message.
When complete, the screen will display Message sent successfully.
