What is PFSH?

In RXNT, PFSH is a commonly used acronym that refers to a section of a patient’s chart containing essential background information, including past medical history, family history, current health status and assessments, and social history.

It provides important context for clinical decision-making and continuity of care, and integrates with the encounter plugin feature so that updates automatically populate into the relevant areas of a patient encounter, ensuring the encounter reflects the most current information.

PFSH is organized into four tabs:
  1. Past history

  2. Family history

  3. Health status / assessments

  4. Social history

Here are some examples of information a practice might keep record of within the PFSH section:
  • Prior surgeries (e.g., appendectomy, knee replacement)
  • Inactive or resolved allergies
  • Family history of disease (heart disease, cancer, diabetes, mental health conditions)
  • Tobacco, alcohol, or substance use
  • Living situation and support system
  • Exercise, diet, and lifestyle habits

Please click PFSH in the navigation menu on the Patient Dashboard.

External source data

Each tab allows users to accept external source data—information entered by the patient through their patient portal. All PFSH components shown here are also available in the portal, enabling patients to provide or update this information as needed. Staff will be alerted when new external source data is available, allowing them to review and accept the information.

Why accept external source data?

Accepting this data adds it to the patient’s chart as part of the medical record. Once added, it will automatically appear within encounters that include the corresponding components (such as Allergies), helping ensure providers have accurate and up-to-date information during visits.

When new External Source Data is available, users will see two indicators:

  1. The PFSH tab in the chart's navigation menu will display a green alert icon
  2. The External Source Data button associated with the component that has new data will appear lighter in color and slightly animated, with a subtle shaking motion to draw attention

In the example below, we walk through a scenario where a patient adds a bee pollen allergy in their portal, and we add it to the PFSH section of their chart. Follow the steps below to learn more.

Click the animated External Source Data button (here, we're clicking the button related to the Allergy component).

The new data will be at the top of the External Data list. Select the new allergy using its checkbox.

Click Add to Active Data.

The allergy will then appear below within the Active Data list. Select its checkbox once more.

Click Review & Accept Allergies to add it to the chart.

The newly accepted allergy will now be listed within the Allergy component of PFSH.

Past history and family history

The Past Hx tab opens by default when accessing the PFSH tab. Here, you can view and enter the following:

  • Medical history
  • Surgical history
  • Hospitalization history
  • Unique Device Identifier(s) for a Patient’s Implantable Device(s)
  • Allergy history
  • Medication history

The Family Hx tab is the second tab and contains only the family history component, which operates identically to the components within Past Hx.

Add data

Each component contains a plus sign icon, allowing you to add new details to each component as needed.

Enter the intended information, then Search.

Click the plus sign to add the data to the component.

The new entry will display at the top of the list. Click Save to complete.

Alternatively, the Medical History, Surgical History, Hospitalization, and Family History sections also offer an option for users to add entries using free text.

If there are no known issues related to a component, select the No Known Significant Problems checkbox (or the corresponding option for that specific component).

Manage data

Users are able to Print, Edit, and Delete data as needed.

To edit an entry, click the file folder icon on a line item. To delete an entry, click the red X icon.

To delete multiple entries, select the intended checkboxes, then Delete above.

Furthermore, use the Other section to keep any notes related to the patient's data.

Health status/assessments

Coming soon!

Click PFSH, then the Health Status/Assessments tab. Here, clinicians can document and review the following health information:

  • Smoking Status
  • Health Concerns
  • Functional Status
  • Cognitive Status
  • Disability Status
  • Pregnancy Status
  • Gravida Para

Social history 

Click PFSH on the side of the Patient Dashboard and click Social Hx. Here, you can view, add, and edit social history that has been entered into RXNT.
