Access the history section
Click More in the header and select History.
Click on the Past, Family, or Social tab to see that history section.
The Headings are updated in the History section for clarity and consistency.
Click on the plus icon to add a new history record.
Fill out the required fields and any additional fields that are relevant.
Click Save to update the history table.
Your provider will automatically receive the medical history reported in your patient portal account.
Click on the trash can icon to remove an incorrect record from your history.
Click Delete to confirm that you want to remove the record.
