Patient portal registration

Available Wednesday, July 24th, 2024, the patient portal registration link and the PIN validation will be able to be sent via text message, in addition to email.

Email registration

Once your provider gives you access to the patient portal, you'll receive an email with a link to register for the portal. Click Register in the email to launch the identity validation process.

The registration link is valid for 72 hours.

Registration validation

Validate your information by entering your last name, date of birth, and email. Click Continue.

PIN verification

You will receive another email once your identity is confirmed. Copy the six-digit portal registration PIN. Check your junk or spam folder if you did not receive the PIN.

Portal

Paste your six-digit portal registration PIN into the patient portal.

Enter

Account details 

Ensure your email address is correct, choose your username and password, then click Create Account.

You will be automatically logged into your patient portal account. Accept the Terms and Conditions.

Terms

Representatives

Those needing access to a patient's portal can register as an authorized representative. Providers can send the patient portal registration link directly to the parent if the patient is a minor, or, patients can invite others to be a representative from within their portal.

Register as a Representative

If you are creating an account as a representative of the patient, select that option while registering and enter your information below, email is required. Choose your username and password. Then, click Create Account.

You will be automatically logged into the patient portal account. Accept the Terms and Conditions.  

Terms

As a representative, you will be prompted to enter the rest of your details. Click Update to save your information.

Add a Representative

Click More from the navigation bar, then select Relations.

Click New in the top right-hand corner.

Fill out the mandatory fields with the asterisks with the information of the representative, then click Send Invite

Relations

The representative will receive an invitation to create an account that will give them access to your personal health information.

Reset a Representative's password

Click More in the header and select Relations.

Click the edit icon for the relation you want to reset.

Click Change Password.

Enter and confirm a new password, then click Submit.

Login to the patient portal

Visit www.rxnt.com/phr. Indicate whether you are the Patient or a Representative using the corresponding radio button.

Enter your Username and Password below, then click Login.

You can also access the patent portal login by visiting www.rxnt.com, and selecting Patient Login.

Access terms and conditions

From the utility icon, you can access the terms and conditions and privacy policy you accepted during registration.

What do I do if I can't login?

If you are receiving an error when logging in to the patient portal, a few steps you can take to troubleshoot are to check the website, select the proper login type, recover your username, and reset your password. If these steps have been taken and you are still unable to login, reach out to your provider's office for assistance. See below instructions for each step!

Check the website

Ensure you are using the correct link for the patient portal (www.rxnt.com/phr).

To be certain, simply navigate to www.rxnt.com and click Patient Login.

Select the proper login type

If the wrong login type is selected, your login credentials will fail. Properly indicate whether you are the Patient or a Representative using the corresponding radio button.

Recover username

Ensure the correct radio button is selected for either Patient or Representative.

Click Forgot Username

Enter your email, last name, and date of birth to confirm your identity, then click Submit.

You must use the same email address that your provider has on file in order to successfully confirm your identity. If you receive a message that your identity can't be confirmed, or if you aren't sure what email is on file for you, reach out to your provider's office.

Reset a forgotten password

Click Forgot Password.

Enter your username and the email associated with your account, then click Continue.

You must use the same email address that your provider has on file in order to successfully reset your password. If you receive a message that your identity can't be confirmed, or if you aren't sure what email is on file for you, reach out to your provider's office.

You'll receive an email with a link to reset your password. The email subject will read RXNT Patient Portal - Password Reset. Click Reset Password.

Check your Spam or Junk folder if you don't receive the email within five minutes.

Your username will automatically populate in the top field. Create your new password, confirm the new password, then click Save.

Reach out to your provider's office

If you are unable to login to the patient portal after attempting the previous four steps, you may need to reach out to your provider's office to verify your identifying information. Confirm your provider has the following identifiers recorded correctly in their system:

  1. Email Address
  2. Last Name
  3. Date of Birth

If your provider's office needs to update any of your identifiers, ensure they have done so prior to attempting the login again.
