Patient portal registration
The patient portal registration link and the PIN validation can now be sent via text message, in addition to email.
Registration link
Once your provider gives you access to the patient portal, you'll receive an email or text message with a link to register for the portal. If you receive the email, click Register in the email to launch the identity validation process. If you received a text message, follow the link provided and tap Register.
The registration link is valid for 72 hours.
Registration validation
Validate your information by entering your Last Name and Date of Birth. Then, select your Communication Preference, choosing between Text or Email.
If you chose email as your communication preference, enter your Email below, then click Continue.
Text
If you chose text as your communication preference, enter your Phone Number below, then click Continue.
PIN verification
Depending on your communication preference, you will receive either an email or text message with the 6-digit verification PIN code.
If you chose email, navigate to your inbox and open the email to obtain the PIN.
Check your junk or spam folder if you did not receive the PIN.
Enter the PIN below.
If you wish to switch your communication preference and receive the PIN by text message, click RECEIVE PIN VIA TEXT.
Enter your phone number, then click SEND PIN.
If you chose text, review your messages to obtain the PIN.
Enter the PIN below.
If you wish to switch your communication preference and receive the PIN by email, click RECEIVE PIN VIA EMAIL.
Enter your email address, then click SEND PIN.
Account details
Ensure your email address is correct, choose your username and password, then click Create Account.
You will be automatically logged into your patient portal account. Accept the Terms and Conditions.
Representatives
Those needing access to a patient's portal can register as an authorized representative. Providers can send the patient portal registration link directly to the parent if the patient is a minor, or, patients can invite others to be a representative from within their portal.
Register as a Representative
If you are creating an account as a representative of the patient, select that option while registering and enter your information below, email is required. Choose your username and password. Then, click Create Account.
You will be automatically logged into the patient portal account. Accept the Terms and Conditions.
As a representative, you will be prompted to enter the rest of your details. Click Update to save your information.
Add a Representative
Click More from the navigation bar, then select Relations.
Click New in the top right-hand corner.
Fill out the mandatory fields with the asterisks with the information of the representative, then click Send Invite.
The representative will receive an invitation to create an account that will give them access to your personal health information.
Update a Representative's information
Click More in the header and select Relations.
Click the edit icon for the relation you want to reset.
Make necessary changes and click Update to save.
Login to the patient portal
Visit www.rxnt.com/phr. Indicate whether you are the Patient or a Representative using the corresponding radio button.
Enter your Username and Password below, then click Login.
You can also access the patent portal login by visiting www.rxnt.com, and selecting Patient Login.

Access terms and conditions
From the utility icon, you can access the terms and conditions and privacy policy you accepted during registration.
