Patients can update their Past Medical, Family, and Social History (PFSH) directly from within their patient portal, making it easier to keep their health record accurate and up to date.
Why is this important?
- Helps providers access the most up-to-date and complete information
- Supports safer, more personalized care and clinical decision-making
- Assists in identifying risk factors from history
- Decreases errors or outdated information during visits
- Saves time during appointments by reducing the need for manual intake updates
Access PFSH
Tap More in the bottom navigation bar.
Tap History.
You can review and manage the following, organized into three tabs:
- Past history (this tab is open by default)
- Family history
- Social history (smoking status)
Past history
Keeping your past medical history up to date and accurate helps your care team better understand your health background and make well-informed decisions about your care.
See past history entries listed here along with their type and status.
Tap on a type of past history to see only entries of that type. Swipe left to see all of the options.
To include more history, like a recent hospitalization for dehydration, just tap Add Past History.
Tap History Type, then select the option that best matches your choice from the menu that appears.
Please enter the event details in the Problem field and the relevant Date, then tap Save.
To remove an entry, tap Delete, then Delete once more. You can only delete entries that you have made. Entries added by your provider cannot be deleted from your side.
Family history
Entering your family history helps your care team identify inherited risks and guide screening and treatment.
Tap the Family tab. Past family history entries are listed here along with the type and status.
Tap the Sort option to view the list by:
- Date
- This arranges the data by the date each entry was recorded, with the newest entries appearing at the top.
- Problem
- This sorts the family history list based on the reported problem.
- Relation
- This organizes the family history list alphabetically by the reported relation.
To include more history, like your father's high cholesterol, just tap Add Family History.
Tap Relation, then select the appropriate family member from the menu that opens.
Please enter the details in the Problem field, along with the appropriate Date. Then tap Save to add the entry.
To remove an entry, tap Delete, then Delete once more. You can only delete entries that you have made. Entries added by your provider cannot be deleted from your side.
Social history
Entering your smoking history and status helps your care team assess health risks, make informed treatment decisions, and provide safer, more personalized care.
Tap the Social tab. You'll see the entries listed here along with their type and status.
Tap the Sort option to view the list by:
- Date
- This arranges the data by the date each entry was recorded, with the newest entries appearing at the top.
- Status
- This arranges the list in alphabetical order by Status.
Tap Add Social History to add an entry.
Tap Smoking status, then select the appropriate response from the menu that opens.
Enter the date relevant to the status, then tap Save.
To remove an entry, tap Delete, then Delete once more. You can only delete entries that you have made. Entries added by your provider cannot be deleted from your side.
