Understanding where to document information and how it flows between modules in RXNT ensures the right team members see the right details at the right time.
If you’re unsure where to document specific information, consider:

Notes and comments in EHR
Patient dashboard notes
Patient dashboard notes are located within the patient chart and are used for communication between clinical staff.
For example, you might note that a patient is anxious about a procedure or needs their blood pressure monitored at the next visit. These notes are only visible within the EHR and are not accessible to patients.
Lab results: notes vs. comments
Lab results contain two separate documentation areas: notes and comments.
If you have a lab integration that sends results directly to RXNT, the interfaced facility has the ability to add notes to a result before releasing it to RXNT. These notes are visible to patients through the patient portal and can also be seen within the EHR. Lab result comments, on the other hand, are added by you or your staff and are located in the Comments section of the lab result. Comments are intended for internal or administrative communication.
For instance, you might document that a result was received late from the lab or that the patient was notified by phone. These comments are not visible to patients and are not considered formal clinical documentation.
Pharmacist notes on prescriptions
Pharmacist notes on prescriptions are entered on prescriptions and provide additional information for the pharmacy. Examples include instructions to counsel a patient on inhaler use or noting a patient’s flavor preferences. These notes are only visible with the EHR on the prescription and are not seen by patients.
PM notes
All notes entered in the PM module are strictly internal and not visible to patients. You have the flexibility to decide where the note is displayed throughout PM.
Patient Dashboard
To add a note to display on multiple screens, navigate to the Patient Dashboard and select the Notes icon on the patient dashboard in either Billing or Scheduling.
Select the screens where you want the note to display:
- Demographic / Patient Dashboard (Billing and/or Scheduling)
- Encounter (Billing)
- Patient Ledger (Billing)
- Patient Payment (Billing)
- Statement (Billing)
- Appointment (Scheduling)
Common examples include reminders to collect an outstanding balance at the next visit, noting that insurance requires prior authorization for future visits, or setting up a payment plan.
Notes entered within specific screens
Notes entered from the Notes icon within specific screens are only visible within that particular screen and are designed for targeted workflows.
Billing screens
Adding a note in Billing-related screens helps document specific actions, for example, adding a modifier due to procedure complexity, recording a partial payment, noting an insurance adjustment, indicating that paper claims should be used for a payer, or documenting updated fee schedules.
