Starting Wednesday, August 17th, 2023, the patient portal registration process will be updated.
New Registration Process
Email registration
Once your provider gives you access to the patient portal, you'll receive an email to verify your identity. Click Register in the email to launch the identity validation process.
Registration validation
Validate your information by entering your last name, date of birth, and email. Click Continue.
PIN verification
You will receive another email once your identity is confirmed. Copy the six-digit portal registration PIN. Check your junk or spam folder if you did not receive the PIN.
Paste your six-digit portal registration PIN into the patient portal.
Account details
Choose your username and password, then click Create Account.
You will be automatically logged into your patient portal account. Accept the Terms and Conditions.
Register as a representative
If you are creating an account as a representative of the patient, select that option while registering and enter your relationship with the patient. Then, click Create Account.
You will be automatically logged into the patient portal account. Accept the Terms and Conditions.
As a representative, you will be prompted to enter the rest of your details. Click Update to save your information.
Add a representative
You can invite others to be a representative on your account. Click More from the navigation bar, then select Relations.
Click New in the top right-hand corner.
Fill out the mandatory fields with the asterisks with the information of the representative, then click Send Invite.
The representative will receive an invitation to create an account that will give them access to your personal health information.
Access terms and conditions
From the utility icon, you can access the terms and conditions and privacy policy you accepted during registration at any time.
