Patient portal registration
Follow the following steps to register for your patient portal account!
- Access registration link
- Validate your identity
- Complete PIN verification
- Complete account details
Access registration link
Once your provider gives you access to the patient portal, you'll receive an email or text message with a link to register for the portal. If you receive the email, click Register in the email to launch the identity validation process. If you received a text message, follow the link provided and tap Register.
The registration link is valid for 72 hours.

Validate your identity
To verify your information, please enter your Last Name and Date of Birth. Then, review and agree to the Terms and Conditions, and complete the reCAPTCHA. When you're ready, click or tap Send Verification Code to continue.
If you received the registration link by email, your email address will be pre-filled below. If you got the link through a text message to complete registration on your mobile device, your phone number will be filled in instead.

Complete PIN verification
Depending on your registration method, you will receive either an email or text message with the 6-digit verification PIN code. Please use the tabs below that match your registration method.
Navigate to your inbox and open the email to obtain the PIN.
Check your junk or spam folder if you did not receive the PIN.
Enter the PIN below. Once done, you will be automatically directed to complete account details.

Complete account details
Depending on how you registered, you may be completing the process on either a desktop or a mobile device. For example, if you received your registration link via text message, you're likely registering on your mobile device. Please select the tabs below that correspond to your registration method.
Enter your Cell Phone number, desired Username, and a Password below. Be sure to confirm the password, ensuring it follows all required criteria. Once done, click Create Account.
Your email address will be pre-filled.
You will be automatically logged into your patient portal account. Accept the Terms and Conditions.

Your cell phone number will be pre-filled.
You’ll get a message confirming your account was created successfully. Please tap the button that matches your device to download the MyRXNT app and access your patient portal account.
Representatives
Those needing access to a patient's portal can register as an authorized representative. Providers can send the patient portal registration link directly to the parent if the patient is a minor, or, patients can invite others to be a representative from within their portal.
Register as a representative
If you are creating an account as a representative of the patient, select that option while registering and enter your information below, email is required. Choose your username and password. Then, click Create Account.
You will be automatically logged into the patient portal account. Accept the Terms and Conditions.
As a representative, you will be prompted to enter the rest of your details. Click Update to save your information.
Add a Representative
Click More from the navigation bar, then select Relations.
Click New in the top right-hand corner.
Fill out the mandatory fields with the asterisks with the information of the representative, then click Send Invite.
The representative will receive an invitation to create an account that will give them access to your personal health information.
Update a Representative's information
Click More in the header and select Relations.
Click the edit icon for the relation you want to reset.
Make necessary changes and click Update to save.
Login to the patient portal
Visit www.rxnt.com/phr.
Patients and their representatives can choose to log in using either their Last Name and Date of Birth along with their preferred contact method for Multi-Factor Authentication (MFA), or by using their username and password.
You can also access the patient portal login by visiting www.rxnt.com, and selecting Patient Login.
Login option 1: Last name, DOB & contact method
This login option will allow users to access all accounts they're connected to (multiple patient accounts and/or representative accounts).
Please enter your Last Name and Date of Birth, then provide your Email address below and click Login. If the details you enter match what's on file at the provider's office, you'll be able to move on to MFA, where a verification code will be sent to your email address.

Login option 2: Username & password
To get started, please let us know if you are the Patient or a Representative by selecting the appropriate button. After that, just enter your Username and Password below, and click Login.
This option does not include MFA.
Forgot password or username?
Ensure you're toggled to the Username & Password login option, then click Forgot Password.
Choose whether you are the Patient or the Representative.
Enter your Username and Email address, then click Reset Password.
You'll get confirmation that an email has been sent to your inbox.
Open the email and click the Reset Password link.
On the Reset Password page, your username will be pre-filled. Just enter your New Password and then Confirm it below. Once they match, go ahead and click Update Password to move forward.
Ensure you're toggled to the Username & Password login option, then click Forgot Username.
Please select if you are the Patient or the Representative.
Provide your Last Name, Date of Birth, and Email address. Once you've filled in the details, just click Submit.
You'll receive a confirmation that an email has been sent to your inbox if there is an account associated with the information you provided.
If you have several patient and/or representative accounts, you will receive an email with all the relevant usernames for your accounts.
Open the email and retrieve the username(s) that were sent to you.
One account
If you have just one portal account, you'll be directed right to your portal dashboard when you log in. If you have multiple accounts, see heading "Multiple accounts" below.
Multiple accounts
If you're a patient or representative registered with several offices through the RXNT patient portal, you can easily access all your accounts with a single login. Once you're logged in, you'll see a list of all the accounts you are connected to. Just remember, this option is available only if you log in using your last name, date of birth, and contact method. If you choose to log in with your username and password, you'll be taken directly to the specific account associated with those credentials.
Molly = Mother
Kelly = Daughter (Minor)
Molly has a patient account for herself and a representative account for her daughter's account.
- Scenario one: Molly chooses login option 1 (last name, date of birth, contact method) and fills in her personal details. After that, she is taken to a listing where she can see and access her own patient account as well as her daughter's account, for which she is a representative. Molly can choose which account she wishes to access by selecting a card from the listing.
- Scenario two: Molly selects login option 1 (last name, date of birth, contact method) and inputs her daughter Kelly's last name and date of birth, but uses her own contact details that are associated with her representative account. Molly is then taken directly into Kelly's account where she is a representative.
- Scenario three: Molly chooses login option 2 (username and password) and enters her username and password for her own patient account. Molly is then taken directly into her own personal patient portal account.
- Scenario four: Molly chooses login option 2 (username and password) and enters her username and password associated with her representative account for Kelly's patient portal. Molly is then taken to her representative account for Kelly.
Review your Patient Accounts and/or Representative Accounts. The most recent account you accessed will be displayed at the top of each list.
Just click on the account you'd like to access. You can easily switch to other accounts from within the patient portal.
Switch account
If you're a patient or representative registered with several offices through the RXNT patient portal, you can easily access all your accounts with just one login. Click on the avatar, then Change Account.
The account you're logged into right now will have a Current tag. Take a look at your other accounts listed and click on one to access it.
In the example below, the user is logged into their Patient Account and has the option to switch to their Representative Account for their daughter.

Access terms and conditions
From the utility icon, you can access the terms and conditions and privacy policy you accepted during registration.
