Sign consent forms

After selecting a patient to check in, consent forms that have been sent to the patient will display for them to sign.

Check-in the patient

Select Check In.

Enter any four-digit PIN twice before handing the device to the patient.

Patient signs consent forms

The patient's consent form(s) will display for them to review and sign. To learn more about sending consent forms to patients, click here. The patient must tap the checkbox to indicate that they agree to sign electronically, enter their name in the text box and tap Sign to complete the form.

After signing all consent forms, the patient will be directed to the next step to confirm their demographics.

Signed consent forms in the EHR

All completed patient forms are saved to the Patient Documents section of the Patient Dashboard under Electronic Patient Forms.
