EHR Patient Management

Starting soon, users will see a refreshed design and an improved experience in the Patient Management features. The update will begin rolling out gradually, starting with some clients on Wednesday, October 8th, 2025, and will be available to everyone in the days that follow.

Search for/modify patients

Within the EHR, the patient search tool can be located in two places:

  1. The global patient search toolbar at the top of the screen, or
  2. The Patient Management section of the utilty menu

Use either to access the Global Patient Search screen.

Use the search fields to locate the intended patient. Search by:

  • Last Name
  • First Name
  • Date of Birth
  • Zip Code
  • Chart Number
  • External Chart Number
  • Preferred Phone Number
  • Account Number
  • or a combination of the available fields

A green flag beside a patient's name in the Global Patient Search indicates that the patient's record has previously been merged.

If only one patient matches your search criteria, their chart will open automatically once you click Search.

If more than one patient matches your search criteria, review the results that populate below, and click the patient's name hyperlink to open their chart.

Within the patient's chart, you may access all demographic and clinical information and make modifications to those components as necessary. 

Merge patients

RXNT provides two ways to merge patients:

  • Deduplication report (recommended for performing bulk merge)
  • Manual patient merge

Create a deduplication report

The RXNT Deduplication report helps you identify potential duplicate patient records in the system and lets you merge these duplicate charts in bulk, which is recommended.

Click Utilities, then expand the Patient Management section and choose Merge Patients.

Click Add New Request to create a list of possible duplicate patients. This will run overnight and review the system for any patient records that might be duplicated.

A notice will appear to inform users when the report was requested and remind them that it will be generated on the next business day.

After the deduplication report is finished, you’ll see a list of all potential duplicates. Click the arrows to the left of each duplicate to expand and review them.

The Primary chart (indicated by the blue toggle) is listed first, followed by one or more secondary matches (indicated by the red check mark).

Duplicates may be flagged if there are conflicts in the two records, such as a mismatched birth date. Determine if a patient match is a duplicate by clicking on the patient's name to review their chart. Click the red check mark to confirm that the records are true duplicates.

Click Confirm to complete this action.

Not a match?

If a record match is not a duplicate, you can mark it by clicking Not a Match. This will remove the records from the merge list and prevent them from appearing as potential matches in future deduplication reports.

Click Confirm to complete this action. The patient will automatically be removed from the merge list.

The icon changes to a green check mark to indicate the record was successfully marked for merge, and it becomes the secondary chart, shown by the gray toggle.

You can use the toggles to switch between the primary and secondary charts if needed. When charts are merged, the primary record's demographics will be kept, and all data from the secondary chart(s) will be added to the primary record.

If you're not sure, just hover over the toggles to see helpful text that confirms which is primary and which is secondary.

Complete single merge within the deduplication report

If needed, you can merge patient records individually from the deduplication report. Make sure the top checkbox is selected, then click Merge to the right of the patient's name.

Complete bulk merge

Bulk merges process overnight.

Once you have reviewed and made your indications for all patients in the merge list, select the top checkbox (next to Group Name) to include all records in the bulk merge.

Click Merge Records.

Review the confirmation details and click Confirm to proceed with the merge.

Alternatively, if you'd like to include only certain records from the report, just select the top checkboxes next to the patient names you want, then click Merge Records at the top of the merge screen. Here's an example:

Just a reminder, after merging, the primary record's demographics will be kept, and all information from the secondary charts will be added to the primary record.

Manual patient merge

It's best to use a manual merge when you have just a few known duplicate patients. For larger numbers, a bulk merge is usually more efficient.

Please click on the Manual Patient Merge tab. Enter some or all of the demographic details for the duplicated patient, then click Search.

Select the checkboxes for the patients to be merged.

The toggles will appear, indicating the primary record (blue toggle), and the secondary record (gray toggle) for the patient. 

Use the toggles to switch between the primary and secondary charts if needed. Remember, when charts are merged, the primary record's demographics will be kept, and all data from the secondary chart(s) will be added to the primary record.

When the patients and primary records have been set, click Send Merge Request. This will process overnight to merge the patient records.

A confirmation message will appear to let you know the merge request was successful.

Merge request status

You can click on the Merge Request Status tab to see all the requested merges and their current statuses.

Click on a batch name to review the merge details.

Unmerge patients merged in error

Use the demographic fields to enter the information for the patient records that were merged by mistake, then click Search.

Click on a batch name to review the request details and ensure the merge needs to be reversed.

To undo a completed patient merge, click Unmerge next to the record, then select Confirm in the confirmation popup.

Deactivate duplicate patients

ONLY for accounts that do not have EHR or ERX subscriptions

If your RXNT subscription includes the Billing solution and/or the Scheduler solution ONLY (i.e., not enrolled for either EHR or ERX), you have the option of deactivating duplicate patient records.

Search for the duplicate patient records and click the name of the patient record to be deactivated.

Click the patient's name to open the demographic editor.

Click  Deactivate.

Click Yes to confirm the patient's deactivation.

Patient records that have been deactivated in error can be reversed by clicking Reactivate in the demographic edit screen.
