After selecting a patient to check-in, intake forms that have been sent to the patient will display for them to sign.
Intake forms replace the previous contact and insurance information screens in the Check-In app. Learn more about using intake forms to update patient demographics.
Check-in the patient
Select Check In.
Enter any four-digit PIN twice before handing the device to the patient.
Patient completes the intake forms
The patient's intake form(s) will display for them to complete. To learn more about sending intake forms to patients, click here.
Starting Tuesday, June 14, 2022, only intake forms sent for the appointment will display to the patient during check-in.
The patient must fill in the answers to the intake form questions and click Complete.
After completing all intake forms, the patient will be directed to the next step to pay their bill.
Completed intake forms in the EHR
All completed patient forms are saved to the Patient Documents section of the Patient Dashboard under Electronic Patient Forms.
